Chelsea Loh - Dr. Johnathan Weinsaft - Week 2

6/10 - 6/14

I spent my second week of immersion shadowing cardiologists and starting on a research project.

I shadowed Dr. Krishnan, an advanced heart failure physician, in the clinic on Monday. During this experience, I learned the importance of taking detailed notes of patient interactions. In the clinic space, physicians follow up on patients weeks or months after their appointment. It would be hard to remember a patient's case or their treatment plan without detailing it in a file. Notes in a medical record would also allow other physicians to view a patient's medical history.

During our weekly Tuesday meetings, Dr. George Shih presented an overview of ChatGPT and its potential role in health care. Dr. Shih showed us an example of AI recognizing a herniation in the spine from an MRI image. Following the meeting, one of the cardiologists on Dr. Weinsaft's team taught me how to analyze cardiac MRI images for a research project. I am now able to identify the chambers of the heart, diastole vs systole, and the valves throughout different slices of MRI images.

I observed three different procedures in the Cath Lab on Wednesday. I observed a stent placement, images taken of the coronary artery, and part of an aortic valve replacement. I wasn't physically in the procedure room, as I did not have a radiation protection vest. However there was a monitor hooked up in the observation area that allowed me to see live imaging. The aortic valve replacement utilized a tool called a 'snake'. Once one of the leaflets of the aortic valve is cut through, the snake would grab the leaflet and pull it out of the way so it doesn't obstruct a coronary artery (from my understanding).

I continued to analyze MRI images on Thursday and Friday. I will be attending Dr. Prince's MRI lecture this afternoon. Next week I will be shadowing Dr. Krishnan in the coronary care unit and will continue MRI analysis, and hope to observe more cath lab or EP procedures.
